Tri-Area Community Health (TACH)

The mission of Tri-Area Community Health (TACH) is to provide affordable community-based health services and to promote good health in our patients and our communities. We carry out our mission by increasing access to affordable health care that is delivered locally, reducing the incidence of preventable illness and injuries, and improving patient self-management of treatable health conditions. The Sliding Fee Program allows Tri-Area Community Health patients who are uninsured or under-insured to receive healthcare services at a lower cost. See website for other locations and information.
