Tiffin Community Health Center, operated by Health Partners of Western Ohio, provides Medical care, Dental Care, Behavioral Health services, Substance Use Treatment, and a full-service Pharmacy. Our mission is to eliminate gaps in health outcomes for all members of our community by providing access to quality, affordable, preventive and primary health care. All patients are welcome at Health Partners, whether you’re insured, uninsured, or underinsured. Prices are based on a sliding fee scale determined by income and family size.
