Vital Records Office – Roosevelt

Birth and death records are filed according to the state where the event occurred. You must bring a valid driver’s license when applying for birth and death certificates. Birth and Death Certificates can be obtained through the TriCounty Health Department in our Vernal office or in Roosevelt. See website for forms needed to bring to TriCounty Health Department. Online Vital Records are also available.
