Advance Community Health

The mission of Advance Community Health is to deliver quality, compassionate primary health care – every patient, every time – in response to the needs of our communities. Our community-based, patient-driven health centers provide a variety of services including primary care, pediatrics, dental care, nutrition education, behavioral health counseling, a 340B pharmacy, mobile health services, health care for the homeless programs, and so much more! Medicaid, Medicare, HMO/PPO, and most commercial insurance plans are excepted. We also offer a Sliding Fee Discount Program for those without insurance that is based on household income.
